This page explains what happens to your information, as a client and as a visitor to this site. Ontario's health privacy law requires me to publish it.¹

Your therapy record

I'm Dennis Liu, Registered Psychotherapist (Qualifying), and I run this practice on my own. Under Ontario's health privacy law (PHIPA), that makes me personally responsible for your record.² I'm registered with the College of Registered Psychotherapists of Ontario (CRPO) and a member of the Canadian Counselling and Psychotherapy Association (CCPA). I follow CRPO's practice standards and CCPA's Code of Ethics, which both cover privacy.

If you become a client, I keep a clinical record, as CRPO and CCPA require. It holds your contact details and background, your consent, and notes about our work. I also keep records of your appointments and payments.³ I use them only to provide your care, handle payment, and meet my legal and professional obligations.

• Your record is kept in Jane, a system built for health care, which encrypts it and stores it in Canada. Online sessions also run through Jane.
• It's kept for at least 10 years after our last contact, then securely destroyed.³
• With your consent, my clinical supervisor, an experienced Registered Psychotherapist, may review it. Supervision is required while I'm a Qualifying registrant.⁴
• Otherwise, nothing is shared without your consent, except in the rare cases where the law requires or allows it, such as a significant risk of serious harm to you or someone else. Your consent form explains them, and we go over them before we start.⁵

Your rights

You can see and get a copy of your record, and ask me to correct it. Email hello@raisedonline.ca and I'll send you a short form to sign. I'll reply within 30 days, or tell you if I need up to 30 more, as the law allows. Copies cost $30 for the first 20 pages, then 25 cents a page, and I'll tell you the cost first.⁶

If your information is ever lost, stolen, or seen by someone who shouldn't have seen it, I'll tell you.⁷

This website

This site never asks for your name or anything about your health.

• Google Analytics shows me how the site is used: which pages people view, how they arrived, and which buttons and links they click. It uses cookies to recognize return visits, and records your device and rough location (city level). Its advertising features are off, detailed data is deleted after 2 months, and Google may process it outside Canada.
• The video doesn't contact YouTube until you press play.
• Booking happens on Jane, not on this site.
